Ultrasound request forms withdrawal
Back in the summer the CCG published the generic ultrasound form, the form was published on the Clinical Support Tool (CST) for SystmOne Practices, and the template was forwarded directly to EMIS practices for immediate use.
Other Provider ultrasound request forms remained in place alongside the new CCG generic ultrasound proforma. We continued to share the generic Ultra sound proforma at e-RS User Group Meetings and shared your feedback with the diagnostic clinical lead. Your comments have been listened to regarding the removal of other Provider forms and we can confirm that with effect from Monday 13 January at 5.00pm both Global Diagnostic and Diagnostic Health Care forms will be withdrawn.
We will keep you informed of a date when we plan to remove the Addenbrookes and NAWFT ultrasound referral forms.
Should you wish to discuss further please do not hesitate to contact me, we would like to thank you for your continued support in using the generic form.
