Expanded Services: Community Oximetry (Sleep Studies) Service

Overview: This service is provided via a Local Enhanced Service (LES) agreement the C&P CCG has in place with a number of GP practices across the C&P CCG area for patients with a sleep related illness who require an assessment and to undertake a community sleep study.

Background: The service originated as a pilot in the Hunts area back in 2012, since expanded to Cambridge and IoE, and more recently to the Wisbech and Peterborough areas.

The aim of this service: Is to deliver an integrated Sleep Study pathway between Primary and Secondary Care (Papworth) that facilitates the shift of the bulk of screening test activity for Sleep Apnoea out into the community/primary care.

How it works: This is a ‘Hub and Spoke’ model, Papworth Hospital being the ‘Hub’, and a number of GP practices being ‘Spokes’. All practices who aren’t signed up to the LES are referred to as ‘Satellite’ GP practices and can refer their patients to any one of the ‘Spoke’ practices of the patients choice.    

The current ‘Spokes’ are as follows:

• Alconbury & Brampton Surgeries – Hunts 
  
• Cedar House – Hunts
  
• The Spinney Surgery – Hunts
  
• Rainbow Surgery – Hunts
  
• Doddington Medical Centre – IoE
  
• Cathedral Medical Centre – IoE
  
• Nuffield Road Medical Centre – Cambridge
  
• Queen Edith Medical Practice – Cambridge
  
• Barley Surgery – Royston
  
• Clarkson Surgery – Wisbech
  
• The New Queen Street Surgery – P’borough
  
• Thistlemoor Medical Centre – P’borough
  
• Park Medical Centre – P’borough

The process:

• Patient presents to GP with sleep related problem and GP suspects Sleep Apnoea.  
  
• GP refers patient (via e-RS) to a ‘Spoke’ practice and sends a letter to the patient with an appointment at a ‘Spoke’ clinic.
  
• Patient attends ‘Spoke’ clinic for further assessment and to collect equipment to carry out an overnight sleep study at home, and then returns equipment to the ‘Spoke’. 
  
• ‘Spoke’ sends overnight sleep study assessment results (electronically) to Papworth for interpretation and analysis. 
  
• Papworth then determines the next steps for the patient, outcomes include:
  
  • Patient discharged back to primary care with no need for further investigation at the Sleep Study Service at Papworth – primary care to manage if required.
  • Or, patient to be onward referred into the Sleep Studies Service at Papworth for further assessment/treatment, via 1st OPA being booked or patient added directly to the Inpatient waiting list.

Post CCT GP Fellowships Programme 2018/19

We are pleased to announce our plans to launch our Post CCT GP Fellowship Programme for 2018/19.  This programme is being developed in collaboration with Health Education England – East of England and the local Acute and Community Trusts.  Our aim is to promote workforce opportunities for newly qualified GPs across Cambridgeshire and Peterborough.
