There is a good evidence base for the role of self-monitoring in achieving blood pressure control.

Patients who self-manage their hypertension achieve lower blood pressures than do those managed with usual care. ([6],[7])

This leaflet has been developed by CAPCCG to provide information to patients and a self-monitoring form.
